Preston Daniel Perez-Mosos

Practice Manager/Co-founder

Hi, I’m Preston Perez-Mosos, the practice manager at our family-owned mental health clinic in Deerfield Beach, Florida. I co-founded the practice alongside my mother, Diana Perez-Nunez, PMHNP-BC, FNP-C, with a shared mission: to create a space where patients are truly cared for—not rushed, not overlooked, and never treated like just another chart. We believe in putting people before profit and always making decisions that prioritize the long-term well-being of our patients—even if that means helping them move on when they no longer need us.

Though I’m not a medical provider, I’ve worked in mental health since I was 19, holding roles in billing, customer service, front desk operations, office management, and public relations. I graduated from Florida Atlantic University in 2022 with a Bachelor’s degree in Neuroscience and Behavior, and I’m excited to begin my journey toward becoming a psychiatric nurse practitioner in late 2025. For now, I continue to support the practice and our patients however I can—often as the first voice you hear when you call our office.
I speak both English and Spanish and strive to make every interaction with our practice warm, respectful, and human. Many of our staff members are like family to me—literally. My mother Diana is the owner and lead provider, Sofia Perez is my cousin, and Izabella Bastos is a lifelong friend. We’ve built this practice on trust, care, and a true sense of community.

Outside of work, I enjoy traveling, attending music events, spending time with friends, and hanging out with my dog. I’m deeply passionate about mental health and grateful for the opportunity to serve every patient who walks through our doors. Thank you for trusting us with your care—it’s an honor we don’t take lightly.

Sofia Perez

Clinical Assistant

My name is Sofia Perez, and I’ve been working at My Wellness Psychiatry for the past year as a Clinical Assistant. In this role, I support providers with clinical documentation, appointment scheduling, and day-to-day administrative coordination. I’m currently pursuing my Master’s in Social Work at Florida Atlantic University, with an expected graduation date of May 2026.

I have prior experience in case management, facilitating support groups, and providing therapeutic support to children and adolescents in the school setting. These experiences have helped shape my passion for mental health, particularly in youth-focused care. I’m especially interested in supporting children and teens navigating anxiety, trauma, identity development, and life transitions.

My long-term goal is to become a licensed clinical social worker (LCSW) and work as a therapist specializing in children and adolescents. I strive to create a safe, inclusive, and culturally sensitive environment where young people feel heard and empowered.
I approach my work with empathy, strong communication skills, and a commitment to trauma-informed, strengths-based care.

Izabella Bastos

Clinical Assistant

With a passion for people and a growing love for medicine, I serve as a Clinical Assistant at our psychiatry practice, where I support both patients and providers through a wide range of responsibilities. My primary focus is on managing clinical documentation, but I also assist patients with appointment scheduling, medication issues, pharmacy coordination, and insurance-related matters.

Before joining the medical field, I worked as an office manager in the kitchen and bath manufacturing industry, where I honed my skills in billing, purchasing, inventory management, and leading a team. Though my career began in a completely different space, I’ve found deep fulfillment in mental health care — so much so, that I’m planning to return to school to become a psychiatric nurse practitioner.
Originally from Brazil and moving to the U.S. at three years old, I grew up bilingual and bicultural. I’m fluent in both English and Portuguese, which helps me connect with a wider range of patients to ensure they feel comfortable and understood throughout their care.

I’ve been with the practice for two years now — and in that time, I also became a mom (twice!). As both a parent and someone who has experienced my own personal mental health challenges, I understand how crucial it is to receive care that’s empathetic, personalized, and free of judgment. With warmth and compassion, I want our patients to know I’m always willing to go that little extra mile for them. I want every person we care for to feel heard, supported, and confident that they’re in good hands.

Nayelis Muniz

Clinical Assistant

I support our providers by helping with clinical documentation, scheduling appointments, and making sure the day-to-day coordination runs smoothly. My goal is to keep things organized so our patients and team can focus on what really matters — providing great care.

I’m currently completing my college degree online, and I bring several years of customer service and phone etiquette experience to my role. That background has taught me the importance of listening, clear communication, and making people feel welcomed.

I believe in creating a friendly and professional environment where every patient feels valued and understood. Whether I’m answering a call, organizing a schedule, or handling important details behind the scenes, I want people to know they can count on me to get things done with care and attention
