My Wellness Psychiatry offers Emotional Support Animal (ESA) evaluations and letters for established clients, at the provider’s discretion. Please note that not all providers within the practice offer ESA letters. The decision to provide an ESA letter is made on a case-by-case basis and is not guaranteed.
New Patients:
If you are a new patient seeking an ESA evaluation, please contact the practice and mention this request during your initial call so we can determine appropriate scheduling.
Current Patients:
If you are an established patient requesting an ESA letter, please discuss this during a follow-up appointment and/orsubmit the requested information below via email.
There is a $75 fee associated with drafting an ESA letter. This fee must be paid prior to the letter being completed.
